Surest Coverage
SUREST COVERAGE FOR SUBSTANCE ABUSE TREATMENT
Surest covers substance abuse treatment as a behavioral health benefit on most employer-sponsored Surest plans. Surest is owned by UnitedHealth Group, the parent company of UnitedHealthcare, and uses the same UnitedHealthcare provider networks and Optum Behavioral Health for behavioral health coverage.
Surest was formerly known as Bind Benefits before UnitedHealth Group acquired it in 2022 and rebranded the product to Surest in 2023. Members who enrolled before the rebrand may still see Bind references in some materials, though Surest is the current product name. The cost structure and network are unchanged through the rebrand.
The defining feature of Surest is its copay-only design. Most covered in-network services have a fixed copay shown upfront, with no annual deductible to meet before benefits start. For substance use disorder treatment, this means you know your out-of-pocket cost per session or per admission before treatment begins.
WHY CHOOSE US
WHAT SUREST PLANS COVER FOR ADDICTION AND MENTAL HEALTH
Surest plans typically cover the standard continuum of addiction and mental health care. Coverage detail varies by employer plan design, but standard coverage includes:
– Medical detoxification, including 24-hour monitored withdrawal management
– Residential treatment, also called inpatient rehab
– Partial hospitalization programs (PHP)
– Intensive outpatient programs (IOP)
– Standard outpatient counseling and therapy
– Dual diagnosis treatment for co-occurring mental health conditions
– Trauma-focused therapy and individual counseling
Surest plans also cover mental health diagnoses such as depression, anxiety, PTSD, and bipolar disorder. Optum Behavioral Health typically authorizes integrated treatment for substance use and co-occurring mental health conditions together.
Under Surest’s copay-only structure, each covered service has a fixed copay shown in the Surest member portal before you receive care. We verify your specific copay amounts during the benefits process.

HOW SUREST'S COVERAGE PROCESS WORKS
Surest’s coverage process combines a traditional behavioral health authorization workflow with its distinctive upfront copay pricing model.
For substance use disorder treatment, Optum Behavioral Health reviews requests using the ASAM Criteria, the addiction medicine standard developed by the American Society of Addiction Medicine. For mental health treatment, Optum applies LOCUS or appropriate clinical criteria for the diagnosis.
For higher levels of care including medical detox, residential treatment, partial hospitalization, and intensive outpatient, Optum requires prior authorization before treatment begins. Your treatment team at Villa Wellness Center submits the authorization request with clinical documentation supporting medical necessity.
Once authorization is in place, Surest’s copay-only structure determines your out-of-pocket cost. Unlike traditional insurance, you do not meet a deductible before benefits start. The fixed copay per service or per day of treatment is shown in the Surest member portal and is collected at the time of service.
HOW TO VERIFY YOUR SUREST BENEFITS WITH VILLA WELLNESS CENTER
We make Surest verification simple and free. Here is the process:
Step 1: Contact us. Call (844) 609-3035 or fill out our insurance verification form verify insurance with your Surest member ID card ready. The card shows your employer group and the UnitedHealthcare network your plan uses.
Step 2: We contact Surest and Optum on your behalf. Our admissions team works with Surest member services and Optum Behavioral Health to confirm covered levels of care, the fixed copay amounts that apply to your plan, and any annual maximums or session limits.
Step 3: We give you a clear cost breakdown. Because Surest uses upfront copay pricing, we can give you an exact out-of-pocket number per session or per admission before treatment begins. This is one of Surest’s distinctive advantages over traditional deductible-based insurance.
There is no cost to verify your Surest benefits and no obligation to enroll. Verification typically takes a few hours to one business day.
HOW SUREST'S COPAY STRUCTURE AFFECTS YOUR COSTS
Surest’s copay-only cost structure is the defining feature that separates it from traditional UnitedHealthcare plans and most other employer-sponsored insurance products.
No annual deductible on in-network covered services. Traditional insurance plans require members to pay a deductible (often several thousand dollars) before benefits begin to share costs. Surest skips the deductible for in-network covered services and starts copay-based cost-sharing immediately.
Fixed copay per service shown upfront. Each covered service has a specific copay amount displayed in the Surest member portal. For substance use disorder treatment, copays may differ by level of care (detox, residential, PHP, IOP, outpatient). The Surest portal shows the amount before you schedule the service.
Annual out-of-pocket maximum still applies. Like all health plans, Surest has an out-of-pocket maximum that caps your total annual cost-sharing. Once you reach the maximum, Surest covers covered services at 100 percent for the rest of the plan year.
Relationship to UnitedHealth Group products. Surest, UnitedHealthcare, and UMR are all UnitedHealth Group products that use the same networks and Optum Behavioral Health. The structural differences: UHC underwrites commercial plans with deductibles and coinsurance, UMR administers self-funded employer plans with employer-determined cost-sharing, and Surest uses the upfront copay-only structure described above
OPTIONS WHEN SUREST COVERAGE IS LIMITED
If your Surest plan has high copay amounts, limited days authorized, or other coverage limitations, you still have options. Federal mental health parity law protects your right to behavioral health benefits at the same level as medical benefits.
We help you understand your appeal rights when Optum denies a request or authorizes a lower level of care than your clinical situation warrants. The appeals process includes internal review by Surest and Optum and external review by an independent reviewer if internal review does not resolve the issue.
For costs not covered by your Surest plan, Villa Wellness Center offers payment plans and self-pay arrangements. Call (844) 609-3035 to discuss your situation.

Does Surest cover the full cost of rehab?
Surest covers rehab as a behavioral health benefit, with cost-sharing through fixed copays rather than deductibles and coinsurance. Most Surest members have some copay per service or per admission. We verify your specific plan to give you exact copay amounts before treatment begins.
Will I need prior authorization for treatment at Villa Wellness Center?
Yes, for most levels of care including medical detox, residential treatment, partial hospitalization, and intensive outpatient. Optum Behavioral Health handles prior authorization for Surest plans, and our admissions team submits the request as part of the admission process.
How long will Surest authorize me to stay in treatment?
Length of authorization varies by clinical need and is reviewed by Optum at set intervals during your treatment. For residential treatment, initial authorization is often for several days at a time, with continued stay reviews based on your progress and ASAM Criteria assessment.
Does Surest cover dual diagnosis treatment?
Yes. Surest typically covers integrated treatment for substance use disorder and co-occurring mental health conditions when both are clinically documented. Optum Behavioral Health authorizes dual diagnosis services together rather than as separate benefits.
What is Surest and is it the same as Bind?
Surest is the current product name. It was previously known as Bind Benefits before UnitedHealth Group acquired it in 2022 and rebranded the product to Surest in 2023. The cost structure, networks, and Optum Behavioral Health administration are unchanged through the rebrand. Members who enrolled under the Bind name may still see Bind references on older materials.
How is Surest different from regular UnitedHealthcare or UMR?
All three are UnitedHealth Group products using the same provider networks and Optum Behavioral Health, but they differ structurally. UnitedHealthcare underwrites commercial insurance plans with traditional deductible and coinsurance cost-sharing. UMR administers self-funded employer plans where the employer determines cost-sharing. Surest uses fixed copays shown upfront with no annual deductible on in-network covered services.
