Surest covers substance abuse treatment as a behavioral health benefit on most employer-sponsored Surest plans. Surest is owned by UnitedHealth Group, the parent company of UnitedHealthcare, and uses the same UnitedHealthcare provider networks and Optum Behavioral Health for behavioral health coverage.

Surest was formerly known as Bind Benefits before UnitedHealth Group acquired it in 2022 and rebranded the product to Surest in 2023. Members who enrolled before the rebrand may still see Bind references in some materials, though Surest is the current product name. The cost structure and network are unchanged through the rebrand.

The defining feature of Surest is its copay-only design. Most covered in-network services have a fixed copay shown upfront, with no annual deductible to meet before benefits start. For substance use disorder treatment, this means you know your out-of-pocket cost per session or per admission before treatment begins.

HOW SUREST'S COVERAGE PROCESS WORKS

Surest’s coverage process combines a traditional behavioral health authorization workflow with its distinctive upfront copay pricing model.

For substance use disorder treatment, Optum Behavioral Health reviews requests using the ASAM Criteria, the addiction medicine standard developed by the American Society of Addiction Medicine. For mental health treatment, Optum applies LOCUS or appropriate clinical criteria for the diagnosis.

For higher levels of care including medical detox, residential treatment, partial hospitalization, and intensive outpatient, Optum requires prior authorization before treatment begins. Your treatment team at Villa Wellness Center submits the authorization request with clinical documentation supporting medical necessity.

Once authorization is in place, Surest’s copay-only structure determines your out-of-pocket cost. Unlike traditional insurance, you do not meet a deductible before benefits start. The fixed copay per service or per day of treatment is shown in the Surest member portal and is collected at the time of service.

HOW SUREST'S COPAY STRUCTURE AFFECTS YOUR COSTS

Surest’s copay-only cost structure is the defining feature that separates it from traditional UnitedHealthcare plans and most other employer-sponsored insurance products.

No annual deductible on in-network covered services. Traditional insurance plans require members to pay a deductible (often several thousand dollars) before benefits begin to share costs. Surest skips the deductible for in-network covered services and starts copay-based cost-sharing immediately.

Fixed copay per service shown upfront. Each covered service has a specific copay amount displayed in the Surest member portal. For substance use disorder treatment, copays may differ by level of care (detox, residential, PHP, IOP, outpatient). The Surest portal shows the amount before you schedule the service.

Annual out-of-pocket maximum still applies. Like all health plans, Surest has an out-of-pocket maximum that caps your total annual cost-sharing. Once you reach the maximum, Surest covers covered services at 100 percent for the rest of the plan year.

Relationship to UnitedHealth Group products. Surest, UnitedHealthcare, and UMR  are all UnitedHealth Group products that use the same networks and Optum Behavioral Health. The structural differences: UHC underwrites commercial plans with deductibles and coinsurance, UMR administers self-funded employer plans with employer-determined cost-sharing, and Surest uses the upfront copay-only structure described above

OPTIONS WHEN SUREST COVERAGE IS LIMITED

If your Surest plan has high copay amounts, limited days authorized, or other coverage limitations, you still have options. Federal mental health parity law protects your right to behavioral health benefits at the same level as medical benefits.
